Iron Studies is a group of blood tests that assess how much iron is in your body and how well it’s being transported and stored. It typically includes serum iron, Total Iron-Binding Capacity (TIBC), transferrin saturation, and often ferritin - together these help diagnose iron-deficiency anaemia or, less commonly, iron overload.
It can help your doctor assess
Iron-deficiency anaemia, anaemia of chronic disease, and iron-overload conditions (haemochromatosis) when levels are unusually high.
This test may be considered for:
Whether this test is right for you depends on your symptoms, medical history and the reason for testing - your doctor can advise.
Serum iron, TIBC, UIBC (unsaturated iron-binding capacity), transferrin saturation percentage, and ferritin (the body’s iron-storage protein).
Serum Iron
Iron Transport
The amount of iron currently circulating in the blood.
TIBC
Iron Transport
Reflects the blood’s total capacity to bind and transport iron - correlates with transferrin levels.
UIBC
Iron Transport
The unused portion of transferrin’s iron-binding capacity.
Transferrin Saturation
Iron Transport
The percentage of transferrin currently carrying iron - low in iron deficiency, high in iron overload.
Ferritin
Iron Storage
Reflects the body’s stored iron reserves - usually the first marker to drop in developing iron deficiency, and the first to rise in overload.
How to read your result
Your report should be read using the reference interval, unit and method shown by the performing laboratory. A result outside a reference interval does not automatically establish a diagnosis, and a result inside the interval does not rule out every health problem. Clinical interpretation depends on symptoms, history and other findings.
| Marker | Result | What it may be associated with |
|---|---|---|
| Ferritin & Serum Iron | Low, TIBC High | Classic pattern of iron-deficiency anaemia - the body is low on iron and trying to bind more of what’s available. |
| Transferrin Saturation | High | May suggest iron overload, requiring further evaluation for conditions like haemochromatosis. |
| Ferritin | High, without high iron | Can occur with inflammation/infection since ferritin also rises as an inflammatory marker - not always a sign of excess iron. |
If a result is outside the range
If the Iron Studies is reported as high, low, positive, negative or otherwise outside the laboratory's stated interval, the meaning depends on the specific test and the reason it was ordered. Your clinician may consider repeat testing, related investigations or other clinical information before deciding what the result means.
Limitations of this test
Ferritin can be falsely normal or high during infection/inflammation even when true iron stores are low, so results are always interpreted alongside CBC and clinical context.
An abnormal result does not by itself mean you have a particular disease. Your doctor interprets results together with your symptoms, history and other tests.
| Parameter | Reference range | Unit |
|---|---|---|
| Serum Iron | 60 - 170 (men), 50 - 170 (women) | µg/dL |
| TIBC | 250 - 450 | µg/dL |
| Transferrin Saturation | 20 - 50 | % |
| Ferritin | 20 - 250 (men), 10 - 120 (women) | ng/mL |
Typical adult ranges for guidance. Reference intervals vary with the laboratory method, age and sex - always read your result against the range printed on your own report.

Prepare
Follow the preparation steps above for this test.
Sample collection
A trained phlebotomist collects the sample at your home - no extra charge.
Laboratory processing
Your sample is processed in our laboratory and reviewed by a pathologist.
Report
Your report is shared on WhatsApp and email, usually within 24 hours.
For a blood-based test, a small blood sample is collected by a trained healthcare professional, usually from a vein in the arm. The collection normally takes only a few minutes; mild temporary discomfort or bruising can occur at the puncture site.
Recent iron supplements, blood transfusion, inflammation/infection (which raises ferritin independent of iron status), and time of day (iron levels vary diurnally) can all affect results.
Laboratory results are interpreted together with your symptoms, medical history and other investigations. If your results are significantly abnormal, or you have symptoms that worry you, discuss them with a qualified doctor promptly.

Fasting (usually 8-12 hours, morning sample preferred) is generally recommended since iron levels vary through the day and after meals - follow your doctor or lab’s specific instructions.
Low ferritin indicates depleted iron stores and is usually the earliest and most reliable marker of iron-deficiency anaemia, often appearing before haemoglobin drops.
Recent iron supplementation can raise serum iron and skew results - inform your doctor if you’re taking iron supplements, as they may advise pausing them briefly before testing.
A CBC shows whether you have anaemia and what type (based on red-cell size), while Iron Studies confirms whether iron deficiency is the specific cause.
Yes, plant-based diets provide a less easily absorbed form of iron, so vegetarians and vegans have a higher risk of iron deficiency, especially women with menstrual blood loss.
It can suggest the body has more iron than it needs, seen in conditions like haemochromatosis - your doctor may order further tests to confirm.
Iron Studies is a blood test. Serum iron, TIBC and ferritin - anaemia workup.
The Iron Studies may be ordered when a healthcare professional needs information about the condition or health area assessed by this test. Iron Studies measure the iron your body stores and transports, giving a fuller picture than haemoglobin alone. It is the standard workup for iron-deficiency anaemia and unexplained fatigue.
Serum iron, TIBC and ferritin - anaemia workup.
The test may be relevant for people whose symptoms, medical history, screening needs or treatment monitoring match the purpose of the Iron Studies.
8 hours fasting recommended.
Yes. Please fast for 8 hours before the Iron Studies. Plain water is usually allowed unless you are told otherwise.
The Iron Studies is done on a blood sample.
For a blood-based test, a small blood sample is collected by a trained healthcare professional, usually from a vein in the arm. The collection normally takes only a few minutes; mild temporary discomfort or bruising can occur at the puncture site.
Reports for the Iron Studies are usually ready within 24 hours of sample collection. You receive them on WhatsApp and email.
The Iron Studies costs Rs 799 at HWP Diagnostic (MRP Rs 1400). Home sample collection is free.

The Iron Studies can provide important information, but laboratory results are generally interpreted together with symptoms, medical history, examination and other investigations. A single result should not be treated as a diagnosis without clinical context.
Some medicines, supplements, food, exercise and other pre-test factors can affect particular laboratory results. Tell the healthcare professional about medicines and supplements you take, and do not stop prescribed medicines unless your clinician tells you to.
